Need a Blank Notary Certificate?

Form Selection Helper

If your document doesn't contain a notarial certificate, you must determine which certificate is required before notarization can be completed. A Notary Public cannot choose a certificate for you or advise which type of notarization your document requires.

Most Common Choice Types

Acknowledgment

Used if you are confirming that you willingly signed a document on your own behalf.

Jurat (Oaths / Affirmations)

Used if you must swear or affirm under oath that the contents of the document are 100% true.

Certified Copy

Used if you need the notary to verify an exact, unedited photocopy of an original document.

Can you tell me which type of notarization I need?

No. A Notary Public cannot provide legal advice or select a certificate for you. Contact the person or organization that prepared, provided, or will receive your document.

Can you prepare or correct my document?

No. We provide notarial services and cannot prepare legal documents, correct legal documents, or provide legal advice.
